Veterans Affairs Secretary David Shulkin told CNN’s Jake Tapper on Monday that he fired the director of Washington D.C.’s Veterans Affairs office within an hour of receiving a negative report on the status of the D.C. Veterans Affairs Medical Center.

“When the Inspector General presented me with an interim report, within one hour I removed the director,” Shulkin said. “In no circumstance am I going to tolerate our veterans being put at risk.”

Shulkin said that he would hold his VA center directors to a “steep” standard of performance.

Tapper listed out the various problems facing the D.C. medical center: “The Inspector General found dirty storage areas, $150 million of equipment not inventoried, no plan to keep track of recalled equipment, not to mention some staffing shortages.”

Shulkin said that preliminary investigations had not turned up these problems in other VA medical centers.
